Y9 Vaccinations and Consent

Dear Parents/Carers of Year 9 students,

We will attend school to vaccinate Y9 students for the Booster dose of DTP Diphtheria, Tetanus & Polio along with Meningococcal ACWY. We will also offer the HPV Human Papilloma Vaccine
if they did not receive this in Y8.

Please copy this link https://www.sheffieldchildrensvaccinations.co.uk/Forms/DTP to complete the consent form for DTP and MENACWY. The option to consent or decline the vaccinations is
at the end of the questionnaire so please complete this even if you do not want your child to have the vaccinations.

Please copy this link https://www.sheffieldchildrensvaccinations.co.uk/Forms/HPV to complete the consent form for HPV, but only if your child did not have HPV last year.

If you consented for HPV last year but your child did not get vaccinated, we will vaccinate them using the consent submitted last year.
