Y9 parents and carers - Immunisation mop up sessions

Dear Parents/Carers of Year 9 students,
 
We will be returning to  Outwood school on 18/06/2024 to vaccinate any Y9 students for the Booster dose of DTP Diphtheria, Tetanus & Polio along with Meningococcal ACWY who missed it last time we were in school. According to our records we are still outstanding a response regarding your child’s consent
 
Please copy this link https://www.sheffieldchildrensvaccinations.co.uk/Forms/DTP to complete the consent form for DTP and MENACWY. The option to consent or decline the vaccinations is at the end of the questionnaire so please complete this even if you do not want your child to have the vaccinations.
